Here are several examples of the phdcc.Data.Form DNN module, each illustrating different aspects of the Form software. There is an option on each form to request a copy of the form results email that we receive.

You can try out the Simple survey form without registering at the site. The other examples require you to register - you can do this using the custom registration form example.
